In the "Set Target Salary Index" field enter the number you want (something in the range of 400 - 600 should be good) and press the "Set" button.

Once you've done that it should raise your salary index by 5 each month until it reaches the target.

Ignore the "Salary Index, Increase by %" field.

You should adjust your automatic settings as well and set them appropriately. The wiki has a good template to go by here: http://simcountry.wikia.com/wiki/Church_Manual
