Beyond requirements for level 3 but still at level 2 (Golden Rainbow)

I've been beyond the requirements for level 3 for some time and will be to level 4 as soon as I get my health index above 100.

I haven't been upgraded to level 3 yet, haven't received my 100 gold coins. Do I have to cash in each country above 1T CS$? That's not stated in the "Documentation" pages that I can find, but I think I saw that somewhere-- can't find it now.

My health index is not responding--still in the low 90's despite having more than needed hospitals. I've just started building war hospital and increasing special clinic--but I shouldn't need them. My rehabilitated workers are at the max allowed and I haven't been to war for some time.

What am I missing in terms of health and level 3 requirements?

I think you need 1T$ in every country.

IIRC you need hospitals ONLY to increase health. Special clinics and war hospitals are counter productive.
